To our Barn Managers,

For vaccine appointments this year, we are requesting each barn manager to provide us with an informaton sheet including the following for each client and patient receiving services:

  1. Client name
  2. Client's current address and phone number
  3. Each horse's name
  4. Vaccines for each horse
  5. Any additional services or medications needed for each horse - ie. Coggins, stool exam, etc.

This will help the front desk staff to verify exactly who is getting services, how many vaccinations are required, and how much time needs to be allotted for each farm call.

Please ensure that we receive yoru information sheet at least a week prior to your allotted appointment time.  If we do not ahve a copy of the information sheet in our office prior to your appointment, we cannot guarantee the requested vaccines or services will be provided.

Thank you for your cooperation and assistance in making these appointments run as smoothly as possible.

Kind regards,
Dr. Steve Miller and Staff
